About the Role

We are seeking an experienced Social Worker to join our team as a Community Care Coordinator in Toronto.

In this role, you will provide rehabilitative services in the community and work as an independent contractor.

Responsibilities

  1. Engage in meaningful dialogue with clients to gather comprehensive assessments of their needs and challenges.
  2. Develop tailored care plans to address specific needs and goals using a client-centered approach.
  3. Set measurable goals to track progress and ensure interventions and services are targeted towards specific outcomes.
  4. Collaborate with multidisciplinary teams to ensure holistic and coordinated approaches to client care.
  5. Provide ongoing support to clients, offering guidance and encouragement to help them navigate challenges and achieve their goals.
  6. Identify, refer, and navigate appropriate community resources to assist patients in reaching their target goals.
  7. Conduct regular follow-up visits to assess client progress, address emerging issues, and adjust care plans as required.

Qualifications

  • Completion of a Bachelor of Social Work from an accredited university (MA of Social Work an asset).
  • Registered with the Ontario College of Social Workers & Social Service Workers.
  • Minimum 5 years of experience working as a Social Worker.
  • Experience working in a community setting is an asset.
  • Valid Ontario driver's license and a reliable vehicle.
  • Proficiency in conducting assessments, interpreting findings, and formulating evidence-based treatment plans.
  • Familiarity with community resources and programs throughout the Toronto Central geography.
  • Proficiency in computer software programs (Microsoft Office) and virtual communication platforms (Microsoft Teams, Zoom, Go To Meeting).

Estimated Salary: $65,000 - $80,000 per year
