Clinical Care Coordinator

About the RoleThis position is part of our Community Commitment Program for Nurses (CCPN), which provides a $25,000 incentive to eligible nurses in exchange for a 24-month commitment to an eligible employer. The CCPN aims to address the nursing shortage in Ontario by providing financial support to nurses who commit to working in communities that need them most.
In this role, you will work as part of a collaborative care team to provide comprehensive client assessments, develop care plans, implement and evaluate care based on best evidence/practices and on client's unique needs. You will also be responsible for supporting clients through care transitions by collaborating and connecting them with community support services in their local community.
Responsibilities- Complete comprehensive client assessments to identify their unique needs and develop personalized care plans
- Implement and evaluate care plans based on best evidence/practices and client feedback
- Support clients through care transitions by connecting them with community resources and services
- Current registration with the College of Nurses of Ontario
- Canadian Nurses Association (CNA) specialty certification an asset
- Enthusiasm and love of community care and passion for evidence-informed practice & excellence in care
- A current CPR certificate is required
- A valid driver's license and vehicle (travel required)
- Updated immunization records, clear criminal background check, and Vulnerable sector check are required upon hire
- A willingness to attain required Saint Elizabeth certifications
