We are seeking a highly skilled Clinical Excellence in Patient Care Coordinator to join our team at Southlake Regional Health Centre. This is an exciting opportunity to work in a dynamic and fast-paced environment, providing exceptional care to our patients and families.

About the Role

As a Clinical Excellence in Patient Care Coordinator, you will be responsible for delivering high-quality patient care, working collaboratively with our healthcare team to ensure seamless delivery of services. Your expertise in clinical excellence will enable you to make informed decisions and take ownership of patient care outcomes.

In this role, you will provide direct patient care to patients based on the three-factor framework as outlined in the College of Nurses of Ontario standard, RN and RPN Practice: The Client, Nurse and the Environment. You will also communicate effectively with patients, families, and all members of the healthcare team.

Key Responsibilities
  • Provide patient and family focused care within the scope of practice, based on the three factor framework of the College of Nurses of Ontario
  • Function in accordance with established hospital policies and procedures, including emergency codes
  • Ensure that practice is consistent with entry to practice competencies, standards of practice, guidelines and legislation
  • Communicate effectively with patients, families and all members of the healthcare team

You will also have the opportunity to contribute to the assessment and documentation of each patient's health status on an ongoing basis, assist in the development and implementation of the Plan of Care and evaluate its ongoing effectiveness.

Requirements
  • RPN diploma from a recognized school of nursing
  • RPN Certificate of Registration, in good standing, from the College of Nurses of Ontario
  • Current BLS certification required
  • Recent related experience in acute general medicine nursing preferred

The estimated salary for this position is between $85,000 to $105,000 per year, depending on your level of experience and qualifications.
