Health Care Social Worker

We are seeking a highly skilled and compassionate Social Worker to join our team at Nova Scotia Health Authority. As a key member of our interprofessional team, you will play a vital role in delivering high-quality care to our patients and communities.
The successful candidate will have a Bachelor's Degree in Social Work and current registration with the Nova Scotia Association of Social Workers. You will have a minimum of two years of experience working in a community-based healthcare setting, with a strong understanding of community-based services and referral processes.
As a Social Worker in Primary Health Care, you will work collaboratively with our team to develop and deliver services to the entire population in the community cluster. You will provide screening, assessment, counseling, and support to patients with chronic disease management distress, as well as facilitate education programs and group meetings.
We are looking for a candidate who is passionate about promoting client-focused care, with a strong commitment to diversity, equity, inclusion, reconciliation, and accessibility. You will have excellent communication and interpersonal skills, with the ability to work effectively with patients, families, public, community partners, and other healthcare professionals.
The successful candidate will be able to work independently and as part of a team, with a strong ability to prioritize tasks, manage time, and resources. You will have a valid driver's license and/or access to reliable transportation, with the ability to travel to various sites within the local network and to meetings in other localities as required.
We offer a competitive salary range of $33.97 - $42.89 per hour, as well as a comprehensive benefits package, including health, dental, travel, long-term disability, and life insurance coverage, as well as a defined benefit pension plan.
