Fraser Health is seeking an experienced Occupational Therapist to join our team in a community health setting. As an Occupational Therapist, you will work with patients to help them achieve their goals and improve their functional independence.
Key Responsibilities- Assess patients' care needs and develop individualized plans to promote functional independence and occupational performance.
- Provide occupational therapy services to patients through assessment, consultation, and interventions.
- Collaborate with community service providers to ensure patient care needs are met.
- Participate in discharge planning rounds and facilitate timely discharge of patients.
- Complete pre-discharge home assessments to aid in transition and discharge planning.
- Current full registration with the College of Occupational Therapists of British Columbia (COTBC).
- Two years' recent, related clinical experience in a community or home health setting.
- Valid BC Driver's License and access to personal vehicle for business-related purposes.
