Job Summary

We are seeking an experienced Occupational Therapist to join our team at Fraser Health. As a Quick Response Occupational Therapist, you will play a critical role in providing occupational therapy services and facilitating discharge planning from acute care facilities to community care services.

Key Responsibilities
  • Provide occupational therapy services to patients through assessment, consultation, and interventions that promote functional independence and occupational performance.
  • Assess patient/client's cognitive, perceptual, self-care, and physical status, and develop individualized plans to address their needs.
  • Collaborate with community service providers to ensure patient/client care needs are met and facilitate timely exchange and provision of information regarding patient/client care.
  • Participate in discharge planning rounds and facilitate timely discharge of patients, ensuring appropriate resources are in place.
  • Identify environmental barriers in the home and make recommendations for modifications to ensure access for patients/clients with disabilities.
  • Prescribe and recommend specialized and adaptive aids and equipment to patients/clients for activities of daily living.
  • Develop and maintain complete documentation of patient/client information, as required.
  • Participate in quality improvement activities and provide input into the development of indicators, standards, practices, and procedures.
Qualifications
  • Current full registration with the College of Occupational Therapists of British Columbia (COTBC).
  • Two years' recent, related clinical experience in a community or home health setting, or an equivalent combination of education, training, and experience.
  • Valid BC Driver's License and access to personal vehicle for business-related purposes.

  • Ability to problem solve and exercise judgment in professional practice.
  • Demonstrated leadership and teaching skills.
  • Ability to work effectively independently and in a team-based environment.
  • Ability to communicate effectively, both verbally and in writing.
  • Ability to deal effectively with conflict.
  • Ability to operate related equipment, including applicable software applications.
