Home Care Coordinator

As a Home Care Coordinator with the Winnipeg Regional Health Authority, you will play a vital role in determining program eligibility and assessing the need for care at home or in a personal care home for elderly or infirm adults. You will coordinate the delivery of professional and non-professional services based on established guidelines.
Key Responsibilities- You will receive referrals and conduct assessments to determine client care needs related to Home Care.
- Perform pattern/needs prioritization and establish appropriate discipline coordinators.
- Develop plans of care for Continuing Care Programs, including statements of client need, objectives, service provision, and evaluation criteria.
- Implement and coordinate care plans, which may involve initiating medical rehabilitation and consultative services as necessary to meet client needs.
- Provide professional interventions where required through counseling/teaching/guidance crisis intervention, etc.
- Maintain current case counts, ensuring proper submission of statistics.
- Plan and organize work schedules and manage caseload demands effectively.
- Utilize consultation and supervision as needed.
- Gather data regarding resources and resource needs related to caseload/community.
- Participate in interpreting the program and resources provided through the Continuing Care Program to the public and/or other agencies.
- Establish and maintain liaison with local healthcare services and informal community resource networks.
- Participate in educational development for new staff, students, and related programs.
- Two years of directly related community experience is preferred.
- Previous experience as a Case Coordinator in Community Health Services is also preferred.
- Experience with EFT implementation is considered an asset.
- A degree in Social Work, Occupational Therapy, Physiotherapy, BSc in Nursing, Registered Practical Nurse, Bachelor of Nursing, Speech-Language Pathology, or related health/human service fields is required.
The estimated salary range for this position is $36.711 to $50.635 per hour, depending on experience and qualifications.
