About the Role:

Vancouver Coastal Health (VCH) is seeking a skilled Licensed Practical Nurse to join their Home Heath team as a Community Health Nurse at Three Bridges Community Health Centre in Vancouver, British Columbia.

Key Responsibilities:

  • Provide direct care to stable and predictable clients experiencing chronic disease, frailty, and/or other conditions that require supports to live independently at home.
  • Use standardized assessment and screening tools to assess, plan, implement and evaluate care, including home support services and accessing community resources.
  • Establish and maintain therapeutic relationships based on mutual respect and client and family-centered care.
  • Support clients and families to achieve and maintain optimal health based on client needs and goals.

About the Salary:

The salary range for this position is estimated to be around CAD $35.08 - $45.44 per hour, depending on experience.
