The Wellness Nurse will be responsible for ensuring the overall well-being of our residents, promoting their health and wellness through various programs and services.
Responsibilities:
- Conduct regular assessments and evaluations of residents to identify their needs and provide appropriate care and services.
- Collaborate with the Resident Care Director (RCD) to develop and implement care plans that meet the unique needs of each resident.
- Monitor and document resident vital signs, including weights, temperatures, and blood pressure.
- Administer medications and treatments as ordered by the attending physician.
- Maintain accurate records and documentation of resident care and services.
- Communicate effectively with residents, families, and healthcare providers to ensure seamless care coordination.
- Participate in quality improvement initiatives to enhance resident care and services.
Qualifications:
- Degree in nursing from an accredited institution.
- Current state license as a Registered Nurse (RN).
- Minimum of one year experience in long-term care or assisted living environment.
- Knowledge of federal, state, and local regulations governing long-term care.
- Excellent communication and interpersonal skills.
- Ability to work independently and as part of a team.
Salary:$65,000 - $75,000 per year, depending on experience.
