Job Summary

We are seeking a skilled Occupational Therapist or Physiotherapist to join our team in a community setting. As a member of our team, you will work closely with clients, healthcare professionals, and other stakeholders to develop and implement personalized rehabilitation plans that promote independence, mobility, and overall well-being.

Key Responsibilities
  • Conduct functional assessments to identify client needs and goals
  • Develop and implement graduated activity and return-to-work plans
  • Liaise with healthcare teams and payer representatives to ensure seamless care coordination
  • Provide education and support to clients and their families to promote independence and self-management
  • Collaborate with other healthcare professionals to develop and implement comprehensive rehabilitation plans
Requirements
  • Master's degree in Occupational Therapy or Physiotherapy
  • Current registration with the relevant professional association
  • Experience working in a community setting, preferably with a focus on rehabilitation and return-to-work
  • Excellent communication and interpersonal skills
  • Ability to work independently and as part of a team
