Job Overview

We are seeking an Advanced Geriatric Care Specialist to join our team. This full-time, permanent position involves providing medical care delivery to LTC residents, managing stable complex chronic co-morbidities in collaboration with MDs, and providing high-quality customer service.

Key Responsibilities
  • Manage stable complex chronic co-morbidities in collaboration with MDs
  • Provide high-quality customer service to residents and their families
  • Deliver clinical/medical education to clients and staff
  • Prioritize quality improvement, education, and leadership responsibilities (approximately 30% of the role)
Requirements and Qualifications

To be successful in this role, you will have:

  • Experience with chronic disease management and geriatric care
  • Clinical experience working with patients with complex health needs
  • Strong communication and interpersonal skills

  • An estimated annual salary of $83,000 based on 8 hours per day, 5 days per week

Work Environment

This role is approximately 70% clinical and 30% quality improvement, education, and leadership. You will work in a dynamic team environment, collaborating with other healthcare professionals to deliver high-quality patient care.
