Nurse Liaison

The Transition Liaison Registered Nurse plays a vital role in enhancing the quality of life for patients and their families by providing comprehensive nursing care and facilitating communication linkages between healthcare services and community partners.
Key Responsibilities:
- Provides nursing care to patients, working independently and as part of an interdisciplinary team to identify and address patient needs and eligibility for services.
- Enhances patient and family experience by consulting, conferring, and collaborating with healthcare providers and professionals.
- Maintains and advances clinical competence through continuous improvement and evidence-based practice.
- Plans, implements, evaluates, and coordinates patient access and care transition activities between healthcare facilities and services across the continuum of care.
- Provides comprehensive assessments of patients and identifies variances from care/discharge plans and service needs.
About this Opportunity:
This is a relief full-time opportunity until January 27, 2026, or the return of the incumbent. Shifts of work will be scheduled days, with shift times of 07:45 to 15:45 hours.
