The Transition Liaison Registered Nurse plays a vital role in enhancing the quality of life for patients and their families by providing comprehensive nursing care and facilitating seamless transitions between healthcare services.
Key Responsibilities:
- Provides nursing care to patients, working independently and as part of an interdisciplinary team to ensure optimal patient outcomes.
- Enhances patient and family experience by consulting, conferring, and collaborating with healthcare providers to address patient needs and eligibility for services.
- Demonstrates continuous improvement and evidence-based practice in nursing care, maintaining and advancing clinical competence.
- Plans, implements, evaluates, and coordinates patient access and care transition activities between healthcare facilities and services across the continuum of care.
- Provides comprehensive patient assessments, identifies variances from care/discharge plans, and resolves patient, caregiver, and system issues.
- Establishes and facilitates communication linkages between healthcare facilities, services, community partners, physicians, health professionals, and patients and their families.
About this Opportunity:
This is a relief full-time opportunity until January 27, 2026, or the return of the incumbent. Shifts of work will be scheduled days, with shift times of 07:45 to 15:45 hours.
