Job Summary

TEEMA Group Solutions is seeking a highly skilled Registered Dietician to provide expert nutrition care and education to our participants. As a key member of our interdisciplinary team, you will be responsible for developing and implementing nutritional care plans, assessing nutritional needs, and communicating nutritional data to our primary care staff.

Key Responsibilities
  • Nutrition Assessment and Planning
    • Collect and analyze patient data to identify nutritional problems and assess satisfaction with nutrition services.
    • Develop and update care plans in collaboration with patients and families, and communicate changes to team members.
  • Nutrition Implementation and Management
    • Ensure that meals meet established standards and guidelines.
    • Provide education and training to stakeholders on nutrition-related topics.
    • Work closely with physicians and other healthcare professionals to ensure seamless care coordination.
    • Document interventions and supervise meal service operations.
  • Nutrition Evaluation and Leadership
    • Monitor nutritional intake and weight changes, and review diet orders to ensure optimal care.
    • Participate in Quality Improvement processes to enhance nutrition services and report food service issues.
    • Actively engage in team meetings and care planning to ensure comprehensive care.
  • Additional Responsibilities
    • Perform additional duties as assigned to support the team and organization.
    Requirements
    • Bachelor's degree in Nutrition from an accredited institution.
    • Minimum 1 year experience as a Dietitian, preferably with a geriatric population.
    • Registered by the Commission on Dietetic Registration.
    • Certified Diabetes Educator (CDE) status preferred, or must be eligible within 24 months.
    • Reliable transportation, valid driver's license, and minimum state-required auto insurance.
    • Medically cleared for communicable diseases and up-to-date immunizations.
    • Pass a comprehensive criminal background check.
    Preferred Qualifications
    • Teamwork and communication skills.
    • Basic computer skills.
    • Ability to teach and provide nutritional guidance to participants and families.
    • Meet competency standards for the position.
    • Medically cleared and immunized before direct participant contact.
    • Pass a background check verifying eligibility for Medicare and Medicaid participation.
