Job Summary

We are currently seeking a skilled Community Mental Health Specialist to join our team at Fraser Health. This role involves working closely with clients and families who are experiencing mental health issues, providing assessment, treatment, and support services. The successful candidate will work collaboratively with our interdisciplinary team to develop and implement comprehensive treatment plans tailored to meet the unique needs of each client.

Key Responsibilities

  • Provide individual, group, and family support to clients with mental health concerns;
  • Assess client needs and develop personalized treatment plans;
  • Liaise with community resources and service providers to ensure seamless care coordination;
  • Advocate on behalf of clients, addressing their rights and interests within the healthcare system;
  • Collaborate with healthcare professionals to facilitate communication and coordination;
  • Document all interactions and progress notes according to established standards and policies;

Requirements

  • Bachelor's degree in a relevant field, such as social work or clinical counselling;
  • Two years' recent, related experience working in mental health; eligibility for registration with relevant associations or boards;
  • Valid driver's license and reliable transportation;
