About the Role

This role involves working collaboratively with our interdisciplinary team to develop individualized care plans and ensure seamless care transitions. As a key member of our healthcare team, you will have the opportunity to make a meaningful impact in the lives of our patients and communities.

Responsibilities
  • To provide direct patient care and develop personalized care plans that cater to the unique needs of each patient.
  • To collaborate with other healthcare professionals to ensure smooth care transitions and optimal patient outcomes.
  • To participate in quality improvement initiatives that aim to enhance patient experiences and health outcomes.
Requirements

For this role, we require:

  • Registration with BC College of Nurses and Midwives as a practicing RN registrant or a practicing RPN registrant (or eligibility).
  • Current CPR Level C certification.
