Healthcare Professional for Patient Care Coordination

Southern Health-Santé Sud seeks a dedicated Healthcare Professional to coordinate patient care in various settings. This role requires proficiency in both official languages (English and French), as well as current registration with the College of Registered Nurses of Manitoba or the College of Registered Psychiatric Nurses of Manitoba.
Responsibilities:- Apply the nursing process to support clients in achieving their optimal health within a people-centered approach.
- Determine work priorities, methods, and actions for unusual matters while maintaining independent judgment.
- Function consistently with Southern Health-Santé Sud's mission, vision, and values.
- Proficiency in both English and French is essential.
- Current registration with the College of Registered Nurses of Manitoba or the College of Registered Psychiatric Nurses of Manitoba, along with defined scope of practice under the Regulated Health Professions Act.
- Current certifications in Basic Life Support, Advanced Cardiac Life Support, Canadian Triage and Acuity Scale, and Trauma Nursing Care Course.
- Knowledge of Professional Standards of Practice and Code of Ethics.
- Able to foster a collaborative interdisciplinary environment, establish positive working relationships, and resolve conflicts effectively.
- Completes and maintains a satisfactory Criminal Record Check, Vulnerable Sector Search, Adult Abuse Registry Check, and Child Abuse Registry Check.
- Immunization is required as a condition of employment according to Southern Health-Santé Sud policy.
