**Job Summary**
We are seeking a highly skilled and compassionate Registered Practical Nurse to join our team as a Home Health Nurse Specialist. In this role, you will provide high-quality nursing care to clients in their homes and communities, working collaboratively with other healthcare professionals to promote health, independence, and quality of life.
**Key Responsibilities**
As a Home Health Nurse Specialist, you will be responsible for:
- Providing comprehensive assessments and developing individualized care plans that address the unique needs and goals of each client.
- Delivering evidence-based nursing care that promotes health, prevents complications, and supports optimal functioning.
- Collaborating with clients, families, and healthcare teams to ensure seamless transitions and continuity of care.
- Developing and maintaining effective relationships with community resources and partners to support client care and well-being.
- Participating in quality improvement initiatives and contributing to the development of best practices in home health care.
**Education, Designations, and Experience**
To be successful in this role, you must have:
- A current registration with the College of Nurses of Ontario (CNO) or equivalent regulatory body.
- Minimum 2 years of experience in community health nursing, with a focus on home health care.
- Demonstrated expertise in assessing and managing complex health conditions, including chronic disease management and palliative care.
- Excellent communication, interpersonal, and leadership skills, with the ability to work effectively with diverse populations and healthcare teams.
**What We Offer**
VON Canada offers a competitive salary range of $85,000 - $110,000 per annum, based on experience and qualifications. This includes a comprehensive benefits package, opportunities for professional growth and development, and a dynamic work environment that values teamwork and collaboration.
