Job Summary:
The Hôtel-Dieu Grace Healthcare is seeking a skilled Nurse Practitioner to join our Addiction Services team. As a key member of our interprofessional team, you will provide comprehensive patient care to individuals struggling with addiction and mental health issues.
Key Responsibilities:
- Provide direct and indirect patient care services, including health assessments, treatment planning, and medication management.
- Collaborate with interprofessional teams to develop and implement comprehensive care plans.
- Work effectively in an interdisciplinary team environment to provide holistic patient care.
- Assess and manage patients with addictions who have co-existing health conditions, including mental health conditions, blood-borne diseases, and infections.
- Provide education to individuals and families about addiction and its management.
- Facilitate and engage in collaborative, inter- and intra-professional, and intersectoral practice when providing care for persons with an addiction(s).
Requirements:
- Current registration with the College of Nurses of Ontario as a Registered Nurse in the Extended Class.
- Minimum of two to three years of community health care and/or primary care experience.
- Master's Degree (Preferred).
- Experience in addiction medicine and harm reduction (Strongly recommended).
- Completion of the 6-hour Suboxone on-line education program (Recommended).
- Current certificate in cardiopulmonary resuscitation (CPR) (Required).
- Successful completion of the Prescribing Narcotics and Controlled Substances course (Copy of certification required).
