Job Summary:

The Hôtel-Dieu Grace Healthcare is seeking a skilled Nurse Practitioner to join our Addiction Services team. As a key member of our interprofessional team, you will provide comprehensive patient care to individuals struggling with addiction and mental health issues.

Key Responsibilities:

  • Provide direct and indirect patient care services, including health assessments, treatment planning, and medication management.
  • Collaborate with interprofessional teams to develop and implement comprehensive care plans.
  • Work effectively in an interdisciplinary team environment to provide holistic patient care.
  • Assess and manage patients with addictions who have co-existing health conditions, including mental health conditions, blood-borne diseases, and infections.
  • Provide education to individuals and families about addiction and its management.
  • Facilitate and engage in collaborative, inter- and intra-professional, and intersectoral practice when providing care for persons with an addiction(s).

Requirements:

  • Current registration with the College of Nurses of Ontario as a Registered Nurse in the Extended Class.
  • Minimum of two to three years of community health care and/or primary care experience.
  • Master's Degree (Preferred).
  • Experience in addiction medicine and harm reduction (Strongly recommended).
  • Completion of the 6-hour Suboxone on-line education program (Recommended).
  • Current certificate in cardiopulmonary resuscitation (CPR) (Required).
  • Successful completion of the Prescribing Narcotics and Controlled Substances course (Copy of certification required).
