Mental Health Clinician for Community Program

Role Summary
The Fraser Health Authority is seeking a highly skilled Clinician to join our Community Mental Health team. As a Mental Health Clinician, you will be responsible for providing assessment, diagnosis, and treatment of individuals with mental health disorders, as well as connecting them with community resources and services.
Key Responsibilities
- Conduct comprehensive assessments of clients to determine their mental health needs and develop individualized treatment plans.
- Provide evidence-based therapeutic interventions, including individual and group therapy, to support clients in achieving their treatment goals.
- Collaborate with community agencies, healthcare providers, and other stakeholders to ensure seamless referrals and transitions of care.
- Maintain accurate and up-to-date client records, including progress notes, treatment plans, and discharge summaries.
- Participate in quality improvement initiatives and contribute to the development of policies and procedures related to mental health services.
Requirements
- Master's degree in Social Work, Clinical Counselling, or a related field.
- Minimum 2 years of experience in a mental health setting, with a focus on community-based services.
- Excellent communication, assessment, and intervention skills.
- Ability to work independently and as part of a multidisciplinary team.
- Valid BC drivers' license and access to a reliable vehicle.
