Social Worker, Home Health Specialist

Fraser Health is seeking a skilled Social Worker to join our Home Health team in Abbotsford, B.C. As a member of our interdisciplinary care team, you will provide therapeutic clinical social work services to clients with significant psychosocial problems.
Key Responsibilities- Conduct psychosocial assessments and provide recommendations for treatment planning and consultation with the interdisciplinary team.
- Provide therapeutic interventions using various modalities, including family counseling and home visits.
- Collaborate with the team to develop and implement care plans for complex client situations.
- Act as a resource and consultant to colleagues and community agencies.
- Facilitate client access to community services and resources.
- Participate in research activities and program planning to improve client care.
- Bachelor's Degree in Social Work from an accredited institution.
- Three years of experience working with clients and caregivers in the community.
- Current registration with the BC College of Social Workers.
- Valid BC Driver's license and access to a personal vehicle.
