Job Title: Social Worker - Mental Health Specialist

Fraser Health Authority is seeking a skilled Social Worker to join our team as a Mental Health Specialist. In this role, you will provide clinical social work counseling services to clients and families with serious, persistent mental illness and substance use in the Assertive Community Treatment (ACT) program.

Key Responsibilities:
  • Conduct comprehensive social work and psychosocial assessments of clients with mental illness and substance misuse/dependence.
  • Provide education and consultation to clients, families, and healthcare professionals on issues related to high-risk factors, recovery, and current community practice in the field of mental health.
  • Develop and coordinate care plans and community services for clients requiring facility placement and/or community support.
  • Collaborate with mental health and related community service agencies and family physicians to determine appropriate levels of care and the effective utilization of client resources.
Qualifications:
  • Masters Degree in Social Work (MSW) from an accredited school of Social Work.
  • Two (2) years' recent, related experience in a mental health treatment setting or an equivalent combination of education, training, and experience.
  • Current full registration with the BC College of Social Workers.
  • Valid Class 5 BC driver's license with access to personal vehicle for business purposes.
