Primary Health Care Manager

We are seeking a highly skilled and experienced Primary Health Care Manager to join our team at Horizon Health Network. As a key member of our leadership team, you will be responsible for overseeing the operations and programming of our Health Centres, ensuring the delivery of high-quality patient care and services.
Key Responsibilities- Plan and evaluate overall operations and programming for Health Centres
- Oversee budget and allocation of resources to respond to Health Centres' needs and population served
- Work with designated leads at each Centre to ensure appropriate staffing
- Sit on the Community Advisory Committee (or as delegate)
- Manage staff-related issues, including attendance management and progressive disciplinary processes
- Ensure clinical practice guidelines, nursing standards, and community standards are met
- Collaborate with providers and staff at community health centres to ensure access to timely care
- Facilitate interdisciplinary team approaches to provide comprehensive care
- Support mental health population health and chronic disease prevention management practices within the teams
- Identify new service requirements and deficiencies in current services, and develop implementation plans
- Participate in recruitment, including job postings, interviews, and orientation
- Approve education requests based on Horizon policy
- Report activities of all Health Centres monthly to the Director
- Complete Performance Appraisals for staff annually
- Work with Medical Liaison(s) and Clinical Department Head of Family Medicine to ensure open communication between medical staff and Horizon
- Maintain good knowledge of community resources to better meet client needs
- Work with Primary Health Care Access Coordinator and other programs to ensure integration of resources and programming
- Conduct team meetings for direction and support
- Serve on Community Health Centres Needs Assessment work groups, as appropriate

Requirements- Bachelor's Degree in a healthcare discipline or related field
- Minimum of 5 years of supervisory experience in healthcare
- Proficiency in Microsoft Office applications
- Demonstrated skills in facilitating, team building, and conflict resolution
- Demonstrated ability in planning, budgeting, information analysis, and reporting
- Experience negotiating services and resources between programs and partners
- Demonstrated knowledge of community partners and resources
- Excellent communications and interpersonal skills
- Demonstrated experience in handling issues and information of confidential and sensitive nature
- Ability to attend work on a regular basis
- Ability to perform the duties of the position
- Good work record
