About the Role

We are seeking a skilled Registered Nurse to join our team as a Navigator. In this role, you will work closely with patients, families, and healthcare providers to ensure seamless care transitions and optimal health outcomes.

Key Responsibilities
  • Conduct comprehensive assessments of patients' needs and develop personalized care plans
  • Collaborate with healthcare teams to coordinate care and services
  • Provide education and support to patients and families on health management and self-care
  • Facilitate communication between patients, families, and healthcare providers
  • Monitor and evaluate the effectiveness of care plans and make adjustments as needed
Requirements
  • Current registration as a Registered Nurse in the province of Ontario
  • Experience working in a healthcare setting, preferably in a navigation or care coordination role
  • Strong communication and interpersonal skills
  • Ability to work independently and as part of a team
  • Proficiency in electronic medical records and other relevant software
