Position Overview

We are seeking compassionate and reliable Community Support Specialists to join our team. In this role, you will play a vital part in assisting individuals with various needs, empowering them to live fulfilling lives within their communities.

Key Responsibilities

  • Provide personalized support to clients, assisting them with daily living activities, social engagement, and accessing community resources.
  • Develop and implement individualized care plans in collaboration with clients, families, and other professionals.
  • Maintain accurate records and documentation of client interactions and progress.
  • Collaborate effectively with a multidisciplinary team to ensure comprehensive client care.
  • Advocate for clients' rights and well-being, connecting them with appropriate services and support systems.

  • Strong interpersonal and communication skills, with the ability to build rapport and trust with clients.
  • Excellent organizational and time management abilities, ensuring efficient delivery of services.
