Job Summary:

The EMP is seeking a skilled and compassionate Nurse Practitioner to join our team. As a key member of our healthcare team, you will be responsible for providing high-quality patient care and collaborating with interdisciplinary teams to achieve optimal patient outcomes.

Key Responsibilities:

  • Assess, plan, implement, and evaluate patient care plans to ensure holistic and comprehensive care.
  • Collaborate with healthcare professionals to develop and implement patient-centered care plans.
  • Provide education and support to patients and their families to promote health and wellness.
  • Conduct thorough assessments of patients' physical and emotional needs, and develop individualized care plans to address these needs.
  • Develop and maintain strong relationships with patients, families, and healthcare teams to ensure seamless care coordination.
  • Stay up-to-date with the latest medical research, technologies, and best practices to provide evidence-based care.
  • Participate in quality improvement initiatives to enhance patient care and outcomes.
  • Communicate effectively with patients, families, and healthcare teams to ensure clear and concise information exchange.

Requirements:

  • Diploma in Nursing or a Bachelor's Degree in Nursing.
  • Active registration with the Nurses Association of New Brunswick.
  • Minimum of two years of specialized nursing experience in family practice, surgical, palliative care, or oncology.
  • Knowledge of the target population of Extra-Mural Services and available community services.
  • Valid New Brunswick Class 5 Driver's License and good driving record.
  • Clean criminal record check.
  • CPR Level C certification.
  • Strong leadership, coaching, and interpersonal skills.
  • Ability to work efficiently as a team member and maintain a good attendance record.
  • High regard for confidentiality.
  • Excellent work habits and professional appearance.
