Mental Health Nurse

We are seeking a highly skilled Registered Practical Nurse to join our Assertive Community Treatment Team (ACTT) in South West. As a key member of our team, you will work collaboratively with clients, families, and community resources to provide comprehensive mental health support.
About the Role:- Conduct mental and physical health assessments, develop plans, and provide treatment and rehabilitation services to clients.
- Work collaboratively with the ACTT team to provide support, assistance, counseling, referrals, and education to clients and their families.
- Monitor and provide follow-up care for clients transitioning out of ACT services.
- Licensed Registered Practical Nurse (Dipl. N.)
- Member in good standing with the Ontario College of Nurses
- Four or more years' experience as an RPN, preferably in a mental health setting
- Excellent health assessment skills, sound clinical judgment, and diagnostic skills
- A second language (French, Cantonese, Mandarin, Farsi, Italian, Korean, Punjabi, Russian, Tamil, Urdu) reflecting the local community is preferred
