About the Role

We are seeking a skilled Registered Practical Nurse to join our Assertive Community Treatment Team in Southwest Program. As a key member of our team, you will work collaboratively with clients, families, and community resources to provide comprehensive care and support.

Key Responsibilities

  • Conduct mental and physical health assessments, develop plans, and provide treatment and rehabilitation services
  • Work collaboratively with the ACT team to manage and administer medication within your scope of practice
  • Provide follow-up nursing care and monitoring to clients being referred to the Clozapine Clinic
  • Conduct mental and physical health assessments, make referrals to community resources, and provide support and education to clients and their families

Requirements

  • Licensed Registered Practical Nurse (Dipl. N.)
  • Member in good standing with the Ontario College of Nurses
  • Four or more years' experience as an RPN, preferably in a mental health setting
  • Excellent health assessment skills, sound clinical judgment, and diagnostic skills
  • A second language reflecting the local community is preferred
  • Knowledge of the Ontario Mental Health Act, mental health reform principles, and PHIPA requirements
