Personal Support Worker

Job Summary:
The Personal Support Worker provides personalized support and homemaking services to clients and families within the framework of VON policies, procedures, and standards. The PSW collaborates with healthcare team members to meet the needs of clients, promoting an optimal level of independence and well-being.
Key Responsibilities:
- Assess and evaluate client needs, developing and implementing personalized care plans to ensure effective support.
- Provide personal care services, including bathing, dressing, and grooming, as well as light housekeeping and meal preparation.
- Support clients with daily activities, such as walking, climbing stairs, and using the bathroom.
- Communicate effectively with clients, families, and healthcare team members to ensure seamless care coordination.
- Monitor and maintain client safety, reporting any concerns or changes to supervisors or healthcare team members.
- Participate in multidisciplinary team meetings to discuss client care plans and progress.
- Complete daily reports and other documentation as required to maintain accurate records.
Requirements:
- High school diploma or equivalent.
- Completion of a Personal Support Worker program or equivalent.
- Current Standard First Aid Certificate or CPR level C Certificate, if applicable.
- Experience working with elderly or ill individuals or children.
- Strong communication and interpersonal skills.
- Ability to work independently and as part of a team.
Working Conditions:
This role requires a detail-oriented approach in a dynamic environment, with physical activity including lifting, carrying, bending, reaching, kneeling, and other movements that emphasize good body mechanics. Individuals in this role are required to walk, sit, stand, and climb stairs throughout the day, with some tasks requiring fine hand movements.
