As a Care Coordinator, you will play a vital role in assessing and determining patient care needs, providing access to community services, and engaging with patients, caregivers, and healthcare practitioners.
Key Responsibilities- Assess patient care needs and determine eligibility for services
- Develop individual care and service plans in collaboration with patients and their families
- Link patients with service providers and coordinate care plan delivery
- Establish a helping relationship with patients and their families, respecting their values and preferences
- Balance patient needs and choices with available resources
- Membership in good standing with the applicable regulatory body
- University degree in Nursing, Physiotherapy, Occupational Therapy, Speech Language Pathology, Social Work
- 2+ years of recent experience in community health or a related field
- Knowledge of the healthcare delivery system and community resources
- Excellent interpersonal, communication, assessment, problem-solving, and decision-making skills
- Effective time management, prioritization, and organizational skills
- Ability to accurately complete required documentation, reports, and forms
- Valid driver's license and access to a reliable vehicle
- Proficient in a Windows environment
- Experience working with diverse patient groups, including multicultural, homeless, palliative, acquired brain injury (ABI), mental health, geriatrics, and pediatrics
- Case management experience or recent related community experience
- Ability to speak French or another second language
