The Palliative Home Care program is an inter-professional team of healthcare professionals dedicated to delivering excellence in palliative care to individuals and families in the community setting. Description:

The Care Manager position is a leadership role within the Palliative Home Care program, reporting directly to the Area Manager of Palliative Home Care. This role supports an inter-professional team of mobile professionals and support staff in the community, ensuring client care and services are delivered using a client-centered approach. The provision of staff support is a fundamental aspect of this role, requiring strong communication and mentoring skills to facilitate internal functions and promote external partnerships.

Key Responsibilities:
  • Support an inter-professional team of mobile professionals and support staff in the community
  • Ensure client care and services are delivered using a client-centered approach
  • Provide staff support and facilitate internal functions
  • Promote and maintain external partnerships
  • Collaborate with Palliative Home Care, Integrated Home Care, and the Palliative/End of Life portfolio leadership team to ensure program-wide objectives are met
Required Qualifications:
  • Demonstrated leadership skills, including decision-making responsibilities, coaching, and teaching
  • Ability to manage human, financial, and physical resources within an operating environment
  • Proven ability to foster partnerships and achieve organizational goals within a unionized sector
  • A demonstrated clear pattern of professional and personal development
Preferred Qualifications:
  • Master's Degree
  • Knowledge related to Palliative Care clinical practices
