About the Role

Vancouver Coastal Health is seeking a highly skilled and experienced Senior Community Health Coordinator to join our team in Vancouver, BC.

The ideal candidate will have a strong background in community health and a passion for delivering high-quality care to individuals and families.

As a Senior Community Health Coordinator, you will be responsible for overseeing and coordinating the delivery of community health services, providing leadership and supervision to designated staff, and evaluating services to ensure program needs are met.

You will also develop policies and procedures, coordinate care and facilitate case planning, and provide clinical and administrative supervision and consultation.

In addition, you will be responsible for promoting Indigenous Cultural Safety and working effectively with community partners and diverse populations.

Key Responsibilities

  • Oversee and coordinate the delivery of community health services
  • Provide leadership and supervision to designated staff
  • Evaluate services to ensure program needs are met
  • Develop policies and procedures
  • Coordinate care and facilitate case planning
  • Provide clinical and administrative supervision and consultation
  • Promote Indigenous Cultural Safety
  • Work effectively with community partners and diverse populations

Required Skills and Qualifications

To be successful in this role, you will require:

  • A Master of Social Work from an accredited School of Social Work
  • Five years' recent related experience providing mental health and substance use clinical services in community-based settings
  • One year of related leadership/supervisory experience
  • Valid BC Driver's License and access to a personal vehicle for business-related purposes
  • Broad knowledge of systems thinking as it relates to social work process
  • Broad knowledge of other health disciplines and their role in healthcare
  • Broad knowledge of research methodology
  • Broad knowledge of supportive and therapeutic counselling skills and clinical practice models
  • General knowledge of Trauma Informed Practice and Harm Reduction approaches and Strengths-based care
  • Demonstrated ability to promote Indigenous Cultural Safety
  • Demonstrated ability to provide competent and culturally safe care in a variety of settings and with diverse populations
  • Demonstrated ability to work effectively with community partners and with individuals from diverse backgrounds

Estimated Salary Range

The estimated salary range for this position is $45.52-$56.85 per hour, based on the location and requirements of the role.
