Intensive Case Manager

We are seeking a highly skilled and compassionate Intensive Case Manager to join our team at Good Shepherd Centres. As a key member of our Residential Services team, you will play a vital role in providing housing support services to individuals and families in need.
Key Responsibilities- Provide trauma-informed intensive case management to clients, including case planning, crisis intervention, and supportive counseling.
- Work collaboratively with clients, community agencies, and other stakeholders to secure permanent housing and provide ongoing support services.
- Develop and maintain relationships with community partners to leverage and coordinate assets and services available in the community.
- Accompany clients to community appointments, apartment viewings, and conduct monthly home visits to ensure their housing and support needs are met.
- Assist clients in a hands-on capacity, as needed, to support their housing stability and well-being.
- Post-secondary education in Social Services or a related field.
- Minimum 1-year related experience or equivalent combination of education, training, and experience in Social Services or a related field.
- Strong knowledge of and experience working with a range of issues impacting individuals and families, including homelessness, poverty, mental illness, and substance use.
- Commitment to and experience working within a person-centered, trauma-informed, harm reduction, and housing first framework.
- Excellent interpersonal and conflict resolution skills, verbal and written communication, time management, and organization.
- Ability to work cooperatively and collaboratively within an interdisciplinary team.
- CPR certification or willingness to be certified.
- Valid G license required.
- Fluency in a 2nd language is an asset.
