We are seeking a highly skilled and compassionate Care Coordinator to join our team at Home and Community Care Support Services. As a Care Coordinator, you will play a vital role in ensuring that our patients receive high-quality, patient-centered care in the comfort of their own homes.
Key Responsibilities- Assess and Determine Patient Care Needs: Conduct thorough assessments to determine the care needs of our patients and develop individualized care plans to meet those needs.
- Link Patients with Service Providers: Connect patients with the necessary service providers to ensure they receive the care and support they need.
- Coordinate and Monitor Care Plan Delivery: Oversee the delivery of care plans and ensure that patients receive the necessary services and support.
- Establish a Helping Relationship with Patients and Families: Build strong relationships with patients and their families to ensure they feel supported and empowered to manage their care.
- Balance Patient Needs and Choices with Available Resources: Work with patients and families to balance their needs and choices with the available resources to ensure that they receive the care and support they need.
- Membership in a Regulatory Body: Be a member in good standing with the applicable regulatory body, such as the College of Nurses of Ontario or the College of Physiotherapists of Ontario.
- University Degree in a Relevant Field: Hold a university degree in a relevant field, such as nursing, physiotherapy, occupational therapy, or social work.
- Experience in Community Health: Have at least 2 years of recent experience in community health or a related field.
- Knowledge of the Healthcare Delivery System: Have a strong understanding of the healthcare delivery system and community resources.
- Excellent Interpersonal and Communication Skills: Possess excellent interpersonal and communication skills to work effectively with patients, families, and healthcare professionals.
- Effective Time Management and Prioritization Skills: Be able to manage time effectively and prioritize tasks to meet deadlines.
- Valid Driver's License and Access to a Reliable Vehicle: Hold a valid driver's license and have access to a reliable vehicle.
- Proficient in a Windows Environment: Be proficient in a Windows environment.
- Experience Working with Diverse Patient Groups: Have experience working with diverse patient groups, such as multicultural, homeless, palliative, acquired brain injury (ABI), mental health, geriatrics, or pediatrics.
- Case Management Experience: Have experience in case management or recent related community experience.
- Ability to Speak a Second Language: Be able to speak a second language, such as French.
