About the Role

The Dementia Support and Education Professional will be responsible for delivering high-quality support and education programs to individuals affected by dementia and their caregivers. This role requires a strong understanding of dementia-related issues, excellent communication skills, and the ability to work independently as part of a regional team.

Key Responsibilities:

  • Develop and implement support and education programs that meet the needs of individuals with dementia and their caregivers.
  • Collaborate with healthcare professionals, allied health professionals, and other stakeholders to build and maintain relationships and expand the referral network.
  • Provide one-to-one telephone support to individuals with dementia and their caregivers, ensuring they have access to information, education, and support services.
  • Maintain accurate client records and statistical reports in accordance with organizational policies and procedures.

About You

To be successful in this role, you will have a post-secondary education or equivalent experience in a health or social agency related field. You will also possess relevant support and education experience, including public speaking, program development, group facilitation, volunteer management, and building relationships with stakeholders.

Requirements:

  • 3-5 years of experience in a health or social agency related field.
  • Extensive knowledge of and experience with dementia-related issues.
  • Strong organizational and time management skills, with the ability to work independently.
  • Flexibility regarding scheduling is expected, with occasional evening and weekend work.
