Key Responsibilities:
- Participate in the provision of patient care, utilizing the Standards of Nursing Practice and the Nursing Process to assess, plan, implement, and evaluate patient care.
- Provide health teaching to patients and their families, promoting health literacy and empowering them to manage their health.
- Collaborate with other healthcare providers to ensure seamless care transitions and optimal patient outcomes.
- Contribute to the development and implementation of patient care plans, ensuring that they are tailored to meet the unique needs of each patient.
- Stay up-to-date with the latest clinical practices and guidelines, applying this knowledge to improve patient care and outcomes.
Requirements:
- Certificate of competency in Ontario
- Current certification in venipuncture
- Telemetry or ECG training
- Previous experience in caring for cardiac, acute stroke, peritoneal dialysis, and oncology patients
Preferred Qualifications:
- Demonstrated patient teaching and discharge planning skills
- Strong critical thinking, problem-solving, time-management, and leadership skills
- Ability to work collaboratively as part of a fast-paced interdisciplinary team
