Job Title: Associate Director of Care/IPAC Lead

Job Summary:

The Associate Director of Care/IPAC Lead is a critical role within our communities, impacting the lives of residents through strong leadership and motivation of Sienna team members.

Key Responsibilities:

  1. Oversee nursing teams based on clearly defined lines of authority and communication.
  2. Perform nursing duties to ensure clinical excellence at all levels in the care community.
  3. Oversee clinical care processes to ensure that care is delivered according to individual plans of care for residents.
  4. Lead the implementation and monitoring of required clinical programs, such as Falls Prevention, Skin and Wound Care, Continence Care, and Pain Management.
  5. Participate in committees according to terms of reference and areas of responsibility.
  6. Model and foster an environment of positive customer service in all aspects of care.
  7. Coordinate and ensure care processes for the RAI MDS Program, Nursing Rehab, and Restorative Care Program are implemented, including evaluation outcomes and monitoring of performance metrics.
  8. Lead recruitment/retention, orientation, and employee labour relations efforts related to Personal Support Workers and Care Support Assistants.
  9. Identify nursing department learning needs and make provisions for appropriate education to support clinical care program delivery and increase capacity building of team members.
  10. Provide team members with regular feedback regarding performance through the probationary period and continuous performance evaluations.
  11. Support home area/neighbourhood leaders with complex care issues or concerns to foster positive resident outcomes.
  12. Assist in the development, monitoring, and evaluation of staffing patterns in accordance with the nursing budget and with Collective Bargaining Agreements as applicable.
  13. Contribute to the development of job routines for all nursing personnel.
  14. Ensure clinical care is delivered in a manner that is resident-centred and fosters evidence-based best practice.
  15. Work with the DOC to ensure nursing practice aligns with the provincial governing body (e.g., College of Nurses) and the organization's vision, mission, and values.
  16. Promote an effective move-in and move-out process.
  17. Participate in Resident and Interdisciplinary Team Conferences as required.
  18. Work in accordance with the organization's Health and Safety Policies and Procedures and in compliance with the Occupational Health and Safety Act/Regulations.
  19. Monitor procurement and supply control procedures to ensure timely availability of resources within the nursing department.
  20. Perform other duties as assigned.

Requirements:

  1. Current registration with the provincial College of Nurses.
  2. A minimum of 3 years' experience in clinical nursing in seniors' care that may include geriatric, long-term care, rehabilitation, or chronic care settings.
  3. Excellent proficiency with WORD, email, internet use, and computerized documentation.
  4. Ability to coach and mentor team members.
  5. Strong customer service experience.
  6. Willingness to upgrade and maintain personal and educational qualifications as required by the changing demands of the job.
  7. As part of legislative requirements, all successful candidates must meet the provincial police check and tuberculosis screening requirements as applicable.

Bonus Points:

  1. Bachelor of Science in Nursing or equivalent post-diploma education preferred.
  2. Knowledge of provincial Long Term Care Act & Regulations preferred.
