Job Description

The Advanced Practice Clinician (APC) plays a vital role in delivering high-quality, patient-centered care within an interprofessional model of care. Working closely with Orthopaedic Surgeons and other healthcare professionals, the APC ensures timely access to musculoskeletal care and provides referrals to appropriate treatment and specialist consultations.

As a champion of best practice, the APC contributes to quality improvement initiatives, driving safe, effective, and efficient care. With an expert knowledge base and sound decision-making skills, the APC possesses the clinical competencies necessary for expanded practice.

Key Responsibilities
  • Perform comprehensive musculoskeletal assessments for select patient populations
  • Establish and communicate clinical impressions within defined scope of practice
  • Initiate communication with and referrals to orthopaedic surgeons based on assessment findings and referral guidelines
  • Review diagnostic imaging to support assessment, diagnosis, and decision-making
  • Provide side-by-side consultations with orthopaedic surgeons and others on the care team
  • Serve as a clinical consultant for the patient population
  • Provide education and recommendations to patients, families, and primary care providers
  • Liaise with referral sources and community service providers as required
  • Contribute to the development, evaluation, and improvement of team-based care and practice innovations
  • Participate in research to address problems and issues arising from practice
Qualifications
  • Registered Health Professional with current registration in Ontario
  • Advanced formal education beyond entry-to-practice training
  • Minimum 5 years clinical expertise in arthritis/MSK diagnoses
  • Completion of the Clinical Practice Skills for Inflammatory Arthritis program
  • Expert knowledge, complex decision-making skills, and clinical competencies
  • Experience in program development and outcome evaluation
  • Collaborative approach to teaching and learning
  • Demonstrated professional leadership, innovation, and team building
  • Demonstrated ability to deal effectively with change
  • Excellent interpersonal, oral, and written communication skills
