Social Services Professional

We are seeking a highly skilled Social Services Worker to join our team at Norview Lodge. As a key member of our multidisciplinary team, you will be responsible for planning, organizing, and implementing programs and services to enhance the quality of life of our residents.
Key Responsibilities- Oversee new resident admissions and provide support to residents and families
- Participate in committees and lead the End-of-Life Committee
- Facilitate education and training for staff and family members
- Coordinate activities and events for Palliative Care week and Pain week
- Provide counseling and support to residents and families
- Lead problem-solving and program planning initiatives
- Maintain accurate and timely documentation
- Participate in interdisciplinary care conferences
- Support Recreation Therapists and participate in CQI initiatives
- Grade 12 plus a Social Services Worker Certificate from a College of Applied Arts and Technology
- Current registration with the Ontario College of Social Workers and Social Service Workers
- Recent experience in long-term care an asset
- Experience in crisis intervention, transitional care, and grief counseling
- Computerized documentation systems and RAI MDS experience
- Knowledge of the Fixing Long-Term Care Homes Act, 2021 and Regulations
- Ability to work independently in a fast-paced environment
- Superior communication, advocacy, and collaboration skills
- Excellent organizational and coordination skills
