Job Title: Renal Social Worker

Lakeridge Health is seeking a highly skilled and compassionate Renal Social Worker to join our team. As a key member of our interdisciplinary team, you will play a critical role in supporting our pre-dialysis patients, providing expertise for our home hemodialysis and peritoneal dialysis programs, and delivering ongoing care to patients in our in-centre hemodialysis units.

Key Responsibilities:
  • Advocate on behalf of patients for community resources
  • Support comprehensive conservative renal care planning
  • Develop goal-oriented treatment plans and interventions
  • Discuss power of attorney and arrange resources and supports
  • Provide patient/family education and support to address psychosocial, emotional, and financial needs
  • Support advance care planning process
Qualifications:
  • Master's degree in Social Work
  • Registration with the OCSWSSW
  • Nephrology Program social work experience required
  • Demonstrated experience with psychological assessments, complex discharge planning, crisis intervention, and adjustment counselling
  • Excellent counselling skills and knowledge of community support services
  • Demonstrated knowledge of and commitment to the principles of chronic disease prevention and management
  • Excellent knowledge of and experience within the healthcare system, particularly related to complex diabetes conditions
  • Experience within a clinic-based setting an asset
  • Demonstrated exceptional interpersonal, verbal, and written communication skills and judgement when relating to adult clients, families, co-workers, and members of the interdisciplinary team
  • Demonstrated ability to build and maintain effective relationships with a diverse range of people and work effectively within an inter-professional team
  • Demonstrated ability to organize workload and establish priorities effectively
  • Demonstrated problem-solving skills and ability to exercise independent judgement and assess situations and address problems efficiently and effectively
  • Motivated self-directed learner, who takes initiative and seeks learning opportunities to enhance client care and professional competence
  • Demonstrated understanding of and commitment to comprehensive safety programs and practices, including staff, client, and environmental safety, and participation in regular in-service education in this area
  • Computer literacy, including knowledge of Meditech and computer programs such as Microsoft Office and Internet Explorer an asset
  • Must have satisfactory performance record

  • Demonstrated understanding of and commitment to Lakeridge Health's comprehensive safety programs and practices, including staff, patient, and environmental safety, and participation in regular in-services in this area
