Occupational Therapist, Assertive Community Treatment Specialist

We are seeking a highly skilled Occupational Therapist to join our Assertive Community Treatment (ACT) team at Fraser Health. As an Occupational Therapist, you will play a vital role in providing direct and indirect rehabilitation services to adults with serious and persistent mental illness.
Key Responsibilities- Assessment and Intervention: Conduct comprehensive assessments of clients' functional capacity, occupational performance status, and environmental supports using recognized occupational therapy and psychosocial rehabilitation tools.
- Rehabilitation Planning: Develop, implement, evaluate, and document individualized rehabilitation plans in collaboration with clients, their support systems, and interdisciplinary team members.
- Direct Intervention: Provide direct individual and group interventions to support clients in achieving their goals, including life skills, education, employment, and leisure programs.
- Consultation and Referral: Provide consultation, referral, and linking support to facilitate clients' effective entry and participation in community-based programs and services.
- Team Collaboration: Participate in interdisciplinary activities, such as client conferences and team meetings, to ensure comprehensive care and support for clients.
- Community Liaison: Act as a consultant/resource to community-based agencies providing rehabilitation services to ACT team clients, ensuring effective delivery of rehabilitation services.
- Program Evaluation: Participate in joint planning with team members, clients, other service providers, and families to identify service needs and determine evidence-based service delivery models within available resources.
- Reporting and Documentation: Provide verbal and written reports regarding progress towards program goals to the team coordinator, including statistical reports and quarterly reports.
To be successful in this role, you will require:
- A Bachelor's degree in Occupational Therapy from an accredited University.
- Two years' recent experience working with psychiatric clients in an acute or community psychiatric setting.
- Current full registration with the College of Occupational Therapists of British Columbia (COTBC).
- Eligibility for membership in the Canadian Association of Occupational Therapists - British Columbia (CAOT - BC).
- A valid B.C. Driver's License and access to a personal vehicle for business use.
You will possess:
- A working knowledge of current psychiatric (psychosocial) rehabilitation and recovery practices.
- Demonstrated knowledge of the major categories of mental illness and their appropriate treatments.
- Knowledge of relevant legislation.
- Demonstrated ability to utilize the analysis and application of activities related to occupational performance.
- Ability to utilize outcome measurement tools.
- Demonstrated leadership and presentation skills.
- Ability to work in an interdisciplinary team environment in a community setting.
- Ability to communicate effectively both verbally and in writing with clients, families, and other members of the healthcare team and community agencies.
- Ability to work effectively with others as well as independently.
- Ability to plan, organize, and prioritize work.
- Physical ability to perform the duties of the position.
- Ability to problem-solve and exercise sound judgment in professional practice.
- Ability to operate related equipment.
