Job Title: Community Nurse

We are seeking a skilled and compassionate Community Nurse to join our team at CarePartners. As a Community Nurse, you will play a vital role in providing wound and ostomy care to patients in the community.

About the Role

This is a full-time/part-time/casual position in the Stratford and surrounding area. As a Community Nurse, you will be responsible for providing wound and ostomy care to patients in their homes, clinics, and community settings. You will work closely with our interdisciplinary team to provide comprehensive care and support to patients and their families.

Key Responsibilities
  • Provide wound and ostomy care to patients in their homes, clinics, and community settings
  • Conduct thorough assessments of patients' wounds and ostomies to identify their needs and develop individualized care plans
  • Implement evidence-based wound and ostomy care practices to promote optimal patient outcomes
  • Collaborate with other healthcare professionals to provide comprehensive care and support to patients and their families
  • Document patient care and progress in a timely and accurate manner
Requirements
  • Registered Nurse (RN) diploma
  • Current College of Nurses of Ontario (CNO) registration in good standing
  • Graduate of Nurses Specialized in Wound, Ostomy & Continence Canada (NSWOCC, formerly CAET), CNA exam may be pending
  • Several years of escalating experience managing wounds and ostomies
  • Strong desire and commitment to making a difference in the lives of our patients
  • Exceptional interpersonal skills
  • Valid driver's license and vehicle as regular travel is required
  • Excellent English verbal and written communication skills
  • CPR Certification
  • Current immunization record including 2 step TB test
  • To protect our patients, we require a current vulnerable sector check and a clear background check
