Job Summary

We are seeking a skilled Nurse Specialized in Wound, Ostomy & Continence (NSWOC) to join our team at CarePartners. As a NSWOC, you will provide wound and ostomy consultation for patients in the clinic and community. If you enjoy caring for a broad range of clients with various wound etiologies and wound care modalities, this is the ideal role for you.

Key Responsibilities

  • Use your knowledge of assessment, prevention, and interventions regarding wound infections and ostomy care
  • Care for patients with consuming wounds, ostomy, continence, provide lower leg assessment including ABPIs
  • Critically appraise the wound care literature and apply research evidence to practice
  • Provide classifications and indications, appropriate utilization to reduce visit frequency and obtain desired outcomes
  • Practice conservative sharp wound debridement (CSWD) and use of other debridement methods
  • Provide edema management including bandaging based on complete lower leg assessment
  • Monitor criteria for adjunctive therapy for complex or recalcitrant wounds
  • Support our patient's safety by practicing according to policy and standards
  • Use your professional assessment skills to perform a thorough holistic assessment of each client and identify the client's needs by utilizing information from the client, caregiver(s), the home environment, the Home and Community Care Support Services (HCCSS) referral and other sources
  • Function as a member of a nursing team who are accountable for the nursing care it provides
  • Develop appropriate time-specific, measurable goals and outcomes in collaboration with the client and/or caregiver(s), and in keeping with the HCCSS goal for treatment

Requirements

  • Registered Nurse (RN) diploma
  • Current College of Nurses of Ontario (CNO) registration in good standing
  • Graduate of Nurses Specialized in Wound, Ostomy & Continence Canada (NSWOCC, formerly CAET), CNA exam may be pending
  • Several years of escalating experience managing wounds and ostomies
  • A strong desire and commitment to making a difference in the lives of our patients
  • Exceptional interpersonal skills
  • A valid driver's license and vehicle as regular travel is required
  • Excellent English verbal and written communication skills
  • CPR Certification
  • Current immunization record including 2 step TB test
  • To protect our patients, we require a current vulnerable sector check and a clear background check
