About the Role

As a Nurse Practitioner at Homewood Health, you will be an integral part of our interdisciplinary team, working collaboratively with consulting physicians, psychiatrists, and other healthcare professionals to provide comprehensive care to patients with complex mental health needs. Your expertise will be invaluable in assessing and treating patients, developing individualized treatment plans, and providing emotional support and guidance to patients and their families.

Key Responsibilities
  • Conduct comprehensive health assessments and develop treatment plans for patients with complex mental health needs
  • Collaborate with consulting physicians and psychiatrists to provide high-quality patient care
  • Support the management of patients with mental health and addiction disorders, including those with PTSD, delirium, dementia, depression, and suicidal ideation
  • Utilize established care paths and order sets to guide care practices for patients from admission to discharge
  • Document patient information accurately and communicate effectively with the healthcare team
Requirements
  • 3+ years of experience as a Nurse Practitioner, with mental health and addiction expertise preferred
  • Current registration in the Extended Class (Nurse Practitioner) with the College of Nurses of Ontario
  • Active membership in related professional associations, such as NPAO and RNAO
  • Mental Health Nursing certificate preferred
  • Masters of Nursing required
