About the Role

St. Joseph's Home Care is a leading provider of home healthcare services, committed to delivering high-quality care and support to individuals in their homes and communities.

This is an exciting opportunity for a skilled and compassionate Registered Nurse to join our team on a full-time basis, working with patients across various settings, including homes and alternative care facilities.

Responsibilities
  • Provide high-quality nursing care to patients with predictable outcomes, within the framework of SJHC's policies and procedures and the standards of practice prescribed by the College of Nurses of Ontario.
  • Maintain knowledge and compliance to the Professional Standards, ensuring adherence to best practices and evidence-based guidelines.
  • Collaborate with multidisciplinary teams, including physical therapists, social workers, and other healthcare professionals, to deliver comprehensive care and support to patients and their families.
  • Conduct thorough assessments, develop, implement, and evaluate individualized care plans, ensuring seamless transitions between care settings.
  • Communicate effectively with patients, families, caregivers, and healthcare teams to ensure informed decision-making and shared understanding of patient needs.
Requirements
  • Bachelor of Science Degree from an accredited university.
  • Current and unrestricted RN registration with the College of Nurses of Ontario.
  • Continence Care Certificate and Basic Cardiac Life Support (BCLS) certification are required.
  • Prior experience in specific nursing care areas, such as palliative, diabetic, or COPD care, is preferred.

Compensation and Benefits

The estimated annual salary range for this position is $60,000-$80,000, commensurate with experience and qualifications. In addition to a competitive salary, we offer a comprehensive benefits package, including health and dental coverage, retirement savings plan, and paid time off.
