Job Title: Registered Nurse/Registered Psychiatric Nurse (RN/RPN) - Long Term CareJob Summary:

Vancouver Coastal Health is seeking a Registered Nurse/Registered Psychiatric Nurse to join the team at Sechelt Hospital in Sechelt, BC. As a member of the interdisciplinary team, you will provide high-quality care to residents in the Long Term Care unit.

Key Responsibilities:
  • Provide nursing assessments and interventions to residents with complex care needs
  • Develop and implement individualized care plans in collaboration with the interdisciplinary team
  • Collaborate with other healthcare professionals to ensure seamless care transitions
  • Provide education and support to residents and their families
  • Participate in quality improvement initiatives to enhance resident care and safety
Requirements:
  • Current practicing registration as a Registered Nurse or Registered Psychiatric Nurse with the BC College of Nurses and Midwives (BCCNM)
  • Completion of an approved gerontology course or an equivalent combination of education, training, and experience
  • Demonstrated ability in nursing practice related to residents who may have chronic diseases
  • Knowledge of pathophysiology of common medical conditions, aging changes, polypharmacy, treatment protocols, and nursing interventions related to common diagnoses and specific care situations
