Job Summary

We are seeking a highly skilled and compassionate Case Assistant to join our Acquired Brain Injury Services team in Langley, B.C. As a key member of our team, you will provide critical support to our clients and families, ensuring they receive the best possible care and services.

Responsibilities
  • Coordinate contract, financial, and administrative aspects of client services and case management under the direction of managers and clinical staff.
  • Support clients accessing services or managing urgent issues in the absence of the case manager and ensure that the case manager is then kept informed; reviews files to obtain historical information and reviews materials such as policy and procedures to assist with various case management issues.
  • Research community resources and identifies information that will be of assistance in the development and implementation of client services and case management plans.
  • Complete initial intake forms for new referrals to the program, as required.
  • Complete forms and arrange for services with other community agencies and Ministries (e.g. Income Assistance).
  • Complete community-based client assessments and assists and/or transports clients to community services. Escorts clients/families to view potential residential placements. Assists in the planning of admissions, discharges, and transfers of clients to residential care placements.
  • Advocate on behalf of clients and families to access system information and resources; promotes client's right to autonomy, dignity and privacy.
  • Organize client service meetings and schedules community appointments for clinical staff.
  • Work collaboratively with team in the development and documentation of care plans. Complete data entry of care plans and other client service documents, contracts, etc. Monitor quarterly reports and other client service documents completed by Contracted Service Providers. Advise case managers of any deficiencies or concerns related to Contracted services.
  • Participate with the trans-disciplinary team in providing input with respect to assessment, development, activation, and evaluation of treatment care plans and programs.
  • Maintain client records by ensuring that client files, applications forms, financial documents, quarterly reports, etc, are kept up to date.
  • Maintain spreadsheets and other client or financial information systems. Complete filing and related administrative duties.
Qualifications

We are looking for a candidate with a Grade 12, Social Services Worker Diploma and two (2) years' recent experience working with children and adults with physical and/or cognitive disabilities or an equivalent combination of education, training and experience. A valid B.C. Driver's License and access to personal vehicle for business-related purposes are also required.
