Job Title: Case Manager

We are seeking a highly skilled and compassionate Case Manager to join our team at The Alex. As a Case Manager, you will play a critical role in providing support and guidance to individuals who are experiencing homelessness and struggling with addiction and mental health issues.

Job Summary:

The Case Manager will work closely with a multidisciplinary team to provide intensive case management services to program participants. This will involve assessing individual needs, developing and implementing personalized service plans, and connecting participants with community resources and services.

Key Responsibilities:
  • Provide culturally sensitive and trauma-informed case management services to individuals experiencing homelessness and addiction.
  • Assess individual needs and develop personalized service plans to address physical, emotional, and social needs.
  • Connect participants with community resources and services, including healthcare, housing, and employment support.
  • Provide ongoing support and guidance to participants, including crisis intervention and conflict resolution.
  • Collaborate with the multidisciplinary team to ensure comprehensive and coordinated care.
  • Maintain accurate and up-to-date records of participant progress and service delivery.
Requirements:
  • Bachelor's degree in a relevant field, such as social work, psychology, or counseling.
  • Experience working with individuals experiencing homelessness and addiction.
  • Strong knowledge of community resources and services, including healthcare, housing, and employment support.
  • Excellent communication and interpersonal skills, with the ability to work effectively with diverse populations.
  • Ability to work independently and as part of a team, with a high level of flexibility and adaptability.
Working Conditions:

This is a full-time position, working 37.5 hours per week. The successful candidate will be required to work in a fast-paced and dynamic environment, with a high level of energy and enthusiasm. The position requires frequent travel to community locations, as well as occasional evening and weekend work.
