About the Role

CarePartners is seeking a skilled Occupational Therapist to join our dynamic Rehabilitation Team in the Cambridge area. As a key member of our team, you will play a critical role in providing high-quality patient care and contributing to the growth and development of our therapy/rehabilitation program.

Key Responsibilities

  • Provide professional assessment and treatment of patients in the community and in their homes
  • Develop and maintain effective care plans in collaboration with the care team
  • Work with patients to establish and achieve time-specific, measurable goals and outcomes
  • Provide guidance and support to Community Support Workers in the field
  • Collaborate with the interdisciplinary care team to ensure comprehensive patient care
  • Perform other duties as required

Requirements

  • Master's Degree in Occupational Therapy from a recognized university
  • Registration with the College of Occupational Therapists of Ontario
  • Proven computer skills and ability to work with electronic health records
  • Strong assessment and treatment skills, with ability to work effectively in a team environment
  • Excellent English verbal and written communication skills
  • French language skills are an asset
  • Clear Background and Vulnerable Sector Check
