Cardiac Rehabilitation Exercise Coordinator

Job Summary
We are seeking a highly skilled and motivated Cardiac Rehabilitation Exercise Coordinator to join our team at Fraser Health. As a key member of our cardiac rehabilitation program, you will play a vital role in helping patients recover from heart conditions and improve their overall health.
About the Role
This is a full-time position responsible for coordinating exercise schedules, planning and supervising exercise sessions for cardiac rehabilitation patients. You will assess patient medical history, perform diagnostic tests, and develop individual care plans. Your expertise will be essential in leading individual and group exercises, education, counseling, and related patient cardiac rehabilitation support activities.
Duties and Responsibilities
- Assess and evaluate patient functional status and risk factors, developing an exercise prescription by reviewing patient information and collaborating with the healthcare team.
- Coordinate exercise schedules for patients, plan and conduct exercise sessions utilizing case management techniques, and demonstrate exercise equipment to ensure patients achieve maximum benefit.
- Monitor patient symptoms and responses to activities, modifying exercise prescriptions as necessary, and responding to complications such as fainting or heart attack during exercise.
- Counsel patients and their families on lifestyle changes, active living, and exercise program guidelines, promoting healthy habits and independence.
- Collaborate with the interdisciplinary team and community by attending conferences, providing information, and participating in quality assurance/improvement activities.
Requirements
To succeed in this role, you must have:
- A Bachelor's degree in Exercise Science, Kinesiology, Physical Education, Sports Medicine, or Physiology.
- Two years' recent related clinical experience in a cardiac rehabilitation environment or an equivalent combination of education, training, and experience.
- Certification with the American College of Sports Medicine (ACSM)-Certified Clinical Exercise Physiologist (CCEP), current CPR Level 'C' certification, and Advanced Cardiac Life Support (ACLS) certification.
